Parents and Guardians, 
During the 89th Legislative Session, Senate Bill 12 passed which requires schools to get annual parental consent before providing routine physical or mental health services. This includes things like first aid, short-term counseling, vision and hearing screenings, etc… 

Tomorrow, your student will come home with a consent form for you to sign and return to school by Friday, if you prefer to NOT complete the online version. You may click the link below to complete the permission form, and the campuses will have access to your responses. 
Regardless of a “yes” or a “no” please complete the form OR return the completed hard copy that was sent home with your student.
